If there are any other special dances you would like to request, please list them in the memo section at the end of this form. When dedicating a song to someone, specify names and relation to you.

The following are some inter-activities that the DJ could use to entertain guests. Some are fairly obvious and require little leading while other extended dances or games involve more interaction. See the "Inter-activities" link on the Madison Grapevine website for a description of each activity. Only select Yes or No if you are sure you would, or would not, like that activity. If you leave it as a 'maybe', the DJ will choose activities that seem to be appropriate for your guests at that venue. And the DJ will use music appropriate for that activity unless you specify a certain song(s).
